TERMS AND CONDITIONS

If you are not completely satisfied with the product, we'll refund you the purchase price paid, subject to the following terms and conditions:

Offer valid on purchases of Zegerid OTC^® made between 11/15/2011 and 12/31/2012.
Request must be postmarked by 2/28/2013 and received by CMS Refund Center no later than 3/20/2013. Limit one (1) refund per person. Offer is valid for U.S. residents only. Must be 18 years or older for eligibility. Allow 4-6 weeks for processing. Duplicated or altered receipts will not be accepted. Void where prohibited, taxed, or otherwise restricted. Not responsible for late, lost, postage due, or undelivered mail.

Refund amount is limited to the purchase price paid minus any coupons or discounts. Manufacturer and its agent reserve the right to adjust refund amount if a coupon value present on the receipt, believed to be received as a discount, is not circled on the submitted receipt or included on the submitted form or card.

If UPC and valid original sales receipt are not included in the request for refund, the purchase price will not be refunded. UPC code from the actual Zegerid OTC^® carton purchased by you (no copies) must be submitted. No requests from groups, clubs, or organizations will be honored.

• Your Original Store-Identified Register Receipt (no copies) with the Purchase Price of Zegerid OTC^® and the Coupon Value used circled
• UPC code from the actual Zegerid OTC carton (no copies)
• Your name, complete mailing address, e-mail, daytime phone number, purchase price, value of coupon (if any) and discounted purchase price printed on a 3x5 card or on the Refund Form provided below.
